Hazlewood Exemption Application Process

  • Complete the appropriate application form(s)
    • HE-V (veterans who have never used the exemption)
    • HE-D (dependents of veterans who are deceased or 100% disabled)
    • TUHH (transfer unused hours to an eligible child)
  • Provide veteran's DD-214 Member 4 copy
  • Provide proof of dependent relationship (birth certificate, adoption certificate, marriage certificate, tax returns)
  • Provide Certificate of Eligibility letter from VA for service after 9/11/01
